Siemens Mobility and Lufthansa Technik Defense plan to build a hospital train for the German armed forces and NATO allies that could carry more than 150 wounded or sick patients, including those needing intensive care, the two companies said on Nov. 3. The companies signed a memorandum of understanding ahead of the Berlin Security Conference to develop the train in Germany for military medical evacuations and civilian disaster response. An MoU is a statement of intent, not a contract. Neither company said whether the Bundeswehr has ordered the train, how many could be built, what they would cost or when the first one could be ready. The design pairs Siemens' Vectouro passenger coaches with a Vectron X locomotive, or a Vectron X Dual Mode version that can run on electric or diesel power. That lets the train keep moving on lines without overhead wires or when the power supply is cut. Lufthansa Technik Defense would fit the coaches with medical equipment ranging from simple stretcher racks to fully equipped intensive care beds, and the train would carry up to 90 medical and support staff. The medical kit comes from aviation. Lufthansa Technik's aeromedical systems have been used for decades by the German Air Force and other militaries, including to turn the Bundeswehr's A400M transport into what the companies call a "flying intensive care unit." Installing the same standard equipment on the train would let patients move between aircraft and rail without being transferred to a different bed. "Especially in these challenging times of geopolitical uncertainty, the capability for medical evacuation is of high strategic importance, and this applies to both land and air transport," said Michael von Puttkamer, vice president of special aircraft services and defense at Lufthansa Technik. Image by Siemens Mobility The coaches are pressure-tight, which allows them to run on high-speed lines, and both the coaches and the locomotives are built for cross-border operation, so the train would not need to change vehicles at national borders. The companies said the coaches can also be converted back for regular passenger service when they are not needed for evacuations, and are designed to last more than 30 years. "Together with Lufthansa Technik Defense, we are creating a high-performance MedEvac solution that combines state-of-the-art medical care with the advantages of rail transport," said Steffen Bobsien, head of locomotives and coaches at Siemens Mobility. The Bundeswehr expects up to 1,000 wounded, injured and sick personnel a day on NATO's eastern flank in a full-scale war, a figure its medical service has stated publicly, and it rehearsed moving large numbers of casualties during its Medic Quadriga 2026 exercise this year. Since 2022, the German government has planned to convert three ICE 3neo high-speed trains into hospital trains with about 30 intensive care places each in wartime, but a final procurement decision is still pending, and the medical service is now drafting its own requirements for a tender, Military Medicine reported in September. Siemens and Lufthansa Technik are not the only bidders preparing for that competition. Rheinmetall and Alstom have shown a modular MedEvacTrain built from standard flatcars carrying swap-out medical containers. Ukraine's war has shown how much evacuation can depend on rail. Nearly two-thirds of Ukraine's wounded are moved by train and about a third by road, with air evacuation the exception because neither side controls the skies, German defense magazine Sicherheit & Verteidigung reported. Ukraine's state railway has converted dozens of passenger cars into mobile hospitals with intensive care units since 2022.
